Should same-sex couples have access to NHS-funded IVF?

NHS-funded IVF is available to female same-sex couples in England who meet age, BMI, smoking and other criteria, but most Integrated Care Boards require them to first complete 6-12 self-funded cycles of artificial insemination (often IUI with donor sperm) to demonstrate infertility, unlike heterosexual couples who try naturally for two years. Updated NICE guidance from March 2026 defines eligibility after six cycles of artificial insemination or a diagnosed cause, applying regardless of orientation, yet local variation and extra upfront costs persist for same-sex couples. This creates unequal access and higher financial barriers estimated at thousands of pounds before NHS support begins.
